***Notes: All GIC plans are covered. You will bring in your Express-Scripts Prescription Drug Card, complete a Rite Aid form and it will be billed directly to your plan.
(Note: Express-Scripts Prescription Drug Card will have an RxBin, the RxGrp and ID No. on it.)

If you are covered under a spouse and not a GIC plan, please check to make sure flu immunization is a covered service on that plan. If so, it will work the same way.
If you do not have insurance, please request a voucher in the comment section during sign up, they will have that voucher for you on the date you receive your flu shot.

To have these flu clinics run smoothly, it would help to print and complete this form bringing it with you on the date you attend. Also these forms will be available in the schools as well as at the flu clinics.
